Thick yellow nails are fungus, psoriasis, trauma, and aging. Confirm with KOH/culture before a hepatotoxic oral. Tea tree has some topical nail-trial data and a lot of irritation. Vinegar soaks are folklore. Oral antifungals still have the mycologic-cure file. Do not stack tea tree plus oral terbinafine and call the liver a plant problem when it is the drug you needed.

Clinical notes

  • One black nail after trauma is a subungual hematoma, not fungus. A streaked melanonychia is melanoma until proven otherwise.

When this is not enough

  • Suspected melanoma/streak, immunocompromised with aggressive tinea, liver disease before an oral antifungal
