Bladder pain syndrome / IC (adjunct)

Pain with filling, relief with emptying, negative cultures. This is not recurrent UTI and chronic uva-ursi (hydroquinone theoretical) is how people get a different problem. Marshmallow mucilage is a rational soothing sip. Cranberry is a UTI-prevention plant and often irritates BPS. Caffeine/tea can worsen frequency — tea as Camellia is not a treatment here.

Primary plants

  1. Marshmallow (Althaea officinalis)

    Althaea officinalis

Root mucilage as a frequent sip to coat irritated urothelium. Traditional, rational, unproven as disease-modifying.

Typical. Cold maceration 2–5 g as sips. Separate from meds if swallowed in volume.

    Safety. Drug binding.

Supportive plants2
  • Ribwort plantain (Plantago lanceolata)

Typical. P. lanceolata leaf infusion — not psyllium seed.

    Safety. Identity: leaf ≠ psyllium.

  • Oat (Avena sativa)

Colloidal oatmeal sitz for pelvic-floor-adjacent perineal itch/burn if the leftover is external.

    Typical. Sitz as labeled.

    Safety. Not a bladder instillation.

Clinical notes

  • Chronic uva-ursi is not an IC protocol. Cranberry often worsens BPS even when it is reasonable for UTI prevention.

When this is not enough

  • Fever/flank (pyelo), hematuria needing cancer workup, retention, pelvic mass, pregnancy UTI
